We are welcoming new patients in both our Cary and Durham locations.

Schedule and Paperwork

Scheduling your appointment is easy and can be done through the Online Scheduler.  A patient portal will be created for you to schedule future appointments and complete paperwork. 

The paperwork will be emailed to you after you create your patient portal and schedule your first appointment.  If the patient is a minor, the health history, dance/activity history, and injury information can be completed by the dancer or a parent/guardian.  The HIPAA/Consent to Treat forms must signed prior to the appointment by a parent/guardian if the patient is under 18 years old.

Insurance and Self-Pay Options

We are in-network with BlueCross BlueShield and Cigna for physical therapy services.  You can submit your insurance information through the Insurance Portal and we will be happy to verify your coverage and benefits.  

 

For patients who opt to pay out of pocket we offer appointments on an individual basis or as part of a discounted 10-visit package.  Contact us or visit the webpage for the service you are interested in and look for the "Purchase" button for more details.

Your First Appointment

Your first appointment will last about 1 hour.  Please make sure that you have completed the paperwork that was emailed when you scheduled your appointment.
 

An additional COVID-19 Screening Form will be emailed 4 hours prior to your scheduled appointment time.  Please also make sure to complete this form before coming into the office.  This form must be completed for every appointment you attend. 

Once you arrive, you will check in using our contactless system.  This can be done using the QR code in the waiting room or the link on our homepage.   This will let the staff know you have arrived.

We ask that patients who are being seen for hip or knee injuries please wear or bring shorts to their appointments.  All other patients can wear comfortable exercise or clothing that is easy to move in.
